Hybrid LPN Clinical Interviewer

Location: Phoenix, AZ 85040
Pay: $28/hour
Schedule: Monday-Friday, 8:30 AM-5:00 PM MST (AZ Local Time)
Hybrid: Monday & Friday Remote | Tuesday-Thursday In Office
Language: Spanish & English preferred

We are seeking a Bilingual LPN Clinical Interviewer to join our team in a hybrid role focused heavily on outbound telephonic outreach . In this position, you will conduct medication interviews with members, gather information to support Medication Therapy Management (MTM) activities, identify medication-related concerns, and provide appropriate education and support within your scope of practice. The ideal candidate is comfortable making a high volume of outbound calls, working in a metrics-driven environment, navigating multiple systems, and communicating effectively with members in both Spanish and English .

Responsibilities

Conduct High-Volume Outbound Medication Interviews

• Make a high volume of outbound calls to members to conduct telephonic medication interviews.
• Verify member identity and obtain appropriate consent before discussing medication-related information.
• Collect accurate information needed to support Medication Therapy Management (MTM) activities.
• Review and reconcile prescription, OTC, herbal, and supplement medications.
• Ask questions to understand medication adherence, effectiveness, side effects, affordability, and barriers to proper medication use.
• Identify potential medication-related concerns and accurately document findings.

Educate, Support & Escalate When Needed

• Provide medication education in clear, easy-to-understand language within your scope of practice.
• Reinforce appropriate medication administration, dosing, storage, and adherence strategies.
• Recognize urgent, complex, or out-of-scope concerns and escalate them appropriately.
• Partner with pharmacists, care coordinators, and operational teams to support appropriate follow-up.

Document Accurately & Meet Quality Standards

• Document member interactions, assessments, and outcomes in designated systems.
• Follow established clinical workflows, scripts, decision trees, and escalation pathways.
• Maintain documentation that meets clinical, regulatory, and audit standards.
• Meet established productivity, quality, and documentation expectations.
• Participate in quality reviews, coaching, audits, and other performance activities.

Work Efficiently in a High-Volume Environment

• Navigate multiple systems simultaneously, including the MTM platform, call systems, email, and documentation tools.
• Manage outbound outreach, documentation, and follow-up activities efficiently.
• Stay current on commonly used medications and program updates.
• Maintain a strong focus on member experience, accuracy, and medication safety.

Required Qualifications & Skills

• Active LPN/LVN license in good standing.
• Bilingual in Spanish and English.
• Experience in a healthcare and/or telephonic outreach environment.
• Comfortable making high-volume outbound calls and working in a metrics-driven environment.
• Strong verbal and written communication skills in both Spanish and English.
• Ability to work effectively in a hybrid environment, both independently from home and in an office/call center setting.
• Ability to navigate electronic documentation systems and Microsoft Office tools.
• Strong attention to detail and ability to accurately document member interactions.

Preferred Qualifications

• Experience with Medication Therapy Management (MTM) .
• Experience with Transition of Care.
• Experience working with Medicare or Medicaid populations.
• Previous experience in a high-volume outbound healthcare call center environment.

• Monday-Friday, 8:30 AM-5:00 PM MST (Arizona local time)
• Monday: Work from home
• Tuesday-Thursday: In-office in Phoenix, AZ 85040
• Friday: Work from home
• Hybrid position with a strong emphasis on outbound member outreach and call productivity .
• $28/hour
